What type of Adult Care Facility is this?

  1. What type of license does the facility hold?
  2. What levels of care are offered?
  3. How often is the facility inspected by the New York State Department of Health?
  4. Is there a written plan of care for each resident?
  5. Can you have a copy of the contract/agreement?
    Are all of the services listed?
    • Monthly Fee
    • Extra charges
  6. What are the staffing schedules and numbers for the Case Manager and Personal Care Assistants? Days/Evenings/Nights
  7. What credentials do the staff members have?
  8. Can staff handle both scheduled and unscheduled needs that arise?
  9. Does staff schedule and coordinate residents medical appointments?
  10. Is transportation to medical appointments provided?
  11. Does staff coordinate home care visits from a nurse, physical therapist, occupational therapist, etc. if needed?
  12. How are medical emergencies handled? Procedures in place?
  13. Is there staffing to help residents who have memory losses and confusion?
  14. Are residents monitored at night?  Are there bed checks?
  15. Does each resident have a Lifeline?

Assisted Living Models

Currently, there are two different Assisted Living models in New York State:

  • Enriched Housing Program: A licensed Adult Care Facility that provides long-term residential care to adults primarily 65 and older. Enriched Housing includes an individual apartment, some meals, housekeeping, laundry, some supervision, assistance with medications, personal care assistance, case management services and structured activity programs.
     
  • Adult Home: A licensed Adult Care Facility that provides long-term residential care, including room, all meals and snacks, housekeeping, laundry, 24-hour supervision, medication and personal care assistance, case management services, and structured activity programs to people 18 years or older.
